    Full digital X-rays, CBCT, periodontal exam, bite force analysis (T-scan). Photo documentation of every tooth. Sit-down case presentation with itemized estimate.

  2. Phase 2

    Periodontal stabilization

    Visit 2-3 · Month 0-1

    Scaling and root planing (all four quadrants, 2 weeks apart). Re-evaluation at 6 weeks showed pockets reduced from 5-6mm down to 3-4mm.

  3. Phase 3

    Safe amalgam removal — phase 1

    Visit 4-5 · Month 2-3

    Safe-removal (SMART protocol) of upper four amalgams, replaced with bonded composite. Rubber dam, high-volume suction, supplemental oxygen, vitamin C rinse before-and-after. Two visits to keep chair time short.

  4. Phase 4

    Cracked-tooth treatment

    Visit 6-7 · Month 4-5

    Endodontic treatment of #19 and #30 (cracked lower molars). Same-day CEREC zirconia crowns following each root canal. Bite verified post-op.

  5. Phase 5

    Safe amalgam removal — phase 2

    Visit 8-9 · Month 5-6

    Removal of lower three amalgams with the same SMART protocol. Composite restorations placed.

  6. Phase 6

    Occlusal splint

    Visit 10 · Month 7

    Lab-fabricated upper hard occlusal splint to protect new restorations from bruxism. Worn nightly. Re-checked at 30 days.

  7. Phase 7

    Zoom whitening — natural teeth only

    Visit 11 · Month 8

    In-office Zoom whitening on remaining natural enamel. Take-home tray for maintenance.

Outcome

Eight months of staged treatment, seven amalgams replaced, two same-day crowns, full periodontal stabilization, custom occlusal splint, and Zoom whitening. Total out-of-pocket cost was $11,200 after insurance. One-year follow-up shows stable periodontal numbers (3-4mm pockets), all restorations intact, the splint still in nightly use.
